That's not a cogent yes-or-no question because the conjunctive pair "real women and really female" is invalid. If one accepts that gender identity can vary from biological sex (and I am unqualified to assess the current majority psychological opinion that they can), then it does not necessarily follow that a "real woman" is "female." Trans women are not and can never be female. Regardless of any fluidity in gender identity, biological sex, which determines "maleness" and "femaleness," is immutable.
Biological males competing against biological females in athletic competition is inherently unfair. It is often, depending on the sport, significantly dangerous for female competitors. Male puberty conveys significant advantages in strength, heart size, lung capacity, concentration of fast-twitch muscle fibers, bone density, and other aspects relevant to athletics. Hormone Replacement Therapy only reduces these advantages from between 10% and 20% (depending on the individual). There is a large body of rigorous research top back this up. Moreover, only a small fraction of transgender males physically transition (<5%). That number is probably lower for athletes, although I know of no reliable data on that point.
Thus the only remaining "debate" in this matter is a combination of semantics (refer to it as "female sports" instead of "women's sports" perhaps?) and whether the feelings of millions of female athletes are to be valued less than those of a relative handful of transgender male athletes who are willing to seize on an unfair advantage to "win."
"The needs of the many outweigh the needs of the few." Transwomen athletes (males) should either compete against members of their own sex cohort or try and establish separate competitions of trans athletes.
