A doctor here in Australia was recently found guilty of murdering his wife via insulin injection. The only reason he was caught via paper trail a) police found out that obtained insulin by writing a script for insulin for a patient and then taking the insulin without giving it to the patient and b) internet browsing record that showed the doctor googling methods to murder the wife and looking up death by insulin. If none of that slip up in paper trail he would have got away scot free.
Similar a nurse being a serial killer murdering patients with a lethal infusion dose of potassium. No one would have found out of a) she decided to just murder one person instead of multiple people. People worked out that lots of death coincide with her administering an IV infusion and b) despite no potassium being charted up, there were missing bags of potassium on the ward not accounted by doctor writing an appropriate legal prescription
Again an organisation like the CIA could easily escape the paper trail scenario and faked a death by accident
Trump probably should watch out if he ever gets hospitalised in the future as that's where a hypothetical assassination may occur
