Most people figured out as soon as they saw his name listed as "Richard Horne" in the credits after his first appearance. They should have just listed it as "Richard" and saved that his last name was Horne for the episode where he robbed his grandma. That would have made for a cool reveal. They did that with Shelly, not revealing her last name until the episode where they revealed that she had been married to Bobby.

I think this stuff all looked exactly the way Lynch wanted it to look. Lots of these effects are near-direct recreations of some art pieces he's made. (I saw a gallery on reddit that compiled a bunch of them and plenty of them are super close to some of these abstract images we see in the show).
