That happened at a job I had once. There were "nooses" hanging at construction site...

...Investigation found that the "nooses" were loops that pipe fitters used to hold sections of pipe while they were welded, cleaned, etc. There were 8 or 10 sets of them.

I was disappointed in our management when the pipe fitters had to bring in specially fabricated hooks to hold their pipe after that instead of management telling the complainers to go stuff themselves.
