For Ann & Bob. As far as I believe, a death certificate could not be issued unless there was a body. When I was part of Jarrow Family History group, a chap called John Ashburner used to take us out to transcribe the gravestones in the cemeteries in South Tynesde. He wanted every service person named on a gravestone to be noted as he believed they may not be recorded anywhere else. I'm guessing that anyone who 'fell overboard' would be noted in the captain's log (or something) but not necessarily recorded by a registrar. Hope this helps a wee bit.
