The only ones I can think of right now that definitely fit the criteria are Kate Bush's "50 Words For Snow" and Lana del Rey's "Lust For Life". Maybe George Harrison's "Living in the Material World"? Been a while since I listened to it.
There's some where the title track is perhaps the weakest track but still not bad (e.g. Pink Floyd's "Obscured By Clouds" and Belle & Sebastian's "Dear Catastrophe Waitress") and some where the title track is maybe the worst on an already-not-very-good album (e.g. Fountains of Wayne's "Traffic and Weather").
