If you want to call Divine "a woman," go ahead, but Divine's real name was Harris Glenn Milstead and he was a transvestite, not a transsexual. He died in 1988 because he was too fat to live and couldn't breathe while sleeping. God knows what "pronoun" he would identify with today.

But yes, (s)he did actually eat dog poop. That's only the third most disgusting scene in the movie, IMO--number 2 is the guy who pushes his anus out, and number 1 is Divine performing fellatio on his son while he yells "yeah! yeah!" and (s)he keeps making this "echhh! ehhh!! eccchhh!" noises.
