Sometimes it's target fixation. Drivers and bicycle riders should look where the want to go, instead of staring at the obstacle they are trying to avoid.

When I ride my mountainbike on a rocky dirt road, I don't stare at the rocks or cliffs. I visualize a path between all the rocks and I follow that path.

Drivers and cyclists should share the road, but sometimes a lane is too narrow for them to share. When that happens, it's legal for the cyclist to ride in the center of the lane.
