Thought I just saw one, but it was just a sub-thread. So here I am, reminiscing.

It happens because posts are stored with filenames that correspond to Unix epoch timestamps.

So if two people post at the exact same second, two links are appended to the thread, but only one file can be created.

really, could have just appended a 5 char random string to the end, and truncated it off for the rendering engine.
