Thanks to a query posed by one poster in a private message, I searched in the newspapers to identify when the photo of Queen Elizabeth the Queen Mother being greeted by Lady Bessborough was taken.

The photo was not taken at the Guildhall as I initially assumed, but at the Savoy Theatre in March 1939. So this is the 5th identified wearing of the tiara.

Hugh Roberts in The Queen's Diamonds notes that a new frame was made for the tiara in 1937, so I think that, in time, we will find that QEQM wore the tiara much more frequently than previously thought. Roberts said it was perhaps worn only once; we now have 5 separate occasions identified.
