That's an intriguing thought, and the reflections to both sides of her face seem to support it, but like Beth I think the largest part of it is probably the lace of her collar.
But I've found a photogravure which depicts Alexandra wearing small pearl(?) drop earrings, and I think this might be accurate and could explain the two brightest spots just below her earlobes in the actual photo.
