Hard to say without knowing the situation at your previous company, but in our case...

we're a very small company. Depending on the time of year and/or number of large projects we have going on... 8-12 employees total at any given time. No HR (other than the owner and me) and we outsource payroll. That being said, the owner... my boss... was supposed to be looking at the info they sent him before every single payroll. How the flying fuck he missed that for so long is beyond me, and I wanted to keep my job so I didn't call him out on it.
