Best practice at least on the Epic side is that you have a "DR" database setup and completely quarantined away. Production copies over to it something like every other day. That way if you're compromised then you at least have a somewhat recent version to fall back to.

I’ve seen it happen with small practices who use a small IT vendor but not such a large hospital. I wouldn’t be shocked if everyone involved in the database side was fired. Someone should have noticed and said something.
